Background: Perinatal depression (PND) is often under-treated and under-recognized. It has a negative impact on infant development and mother-child interactions. This study aims to estimate the prevalence of PND during pregnancy and in the postpartum period and the effect of sociodemographic factors, psychosocial stressors, and obstetric and neonatal factors on PND. Methods: 166 antenatal mothers attending tertiary center, who completed the 1st-trimester, were evaluated on baseline sociodemographic, psychosocial, obstetric, neonatal, and post neonatal factors by using a semi-structured questionnaire. Periodic prospective assessments were done using Hamilton depression rating scale (HAMD) at the end of the second and third trimesters and first and sixth weeks of the postpartum period. Results: Prevalence of PND was 21.7%, 32.2%, 35%, 30.4%, and 30.6%, at the end of the first trimester, during second, and third trimesters, and first and sixth week postpartum, respectively. Factors significantly associated with depressive symptoms included history of previous children with illness (P: 0.013, OR-5.16, CI-1.3-19.5) and preterm birth (P: 0.037, OR-3.73, CI-1.1- 13.2) at the time of recruitment; history of abuse (P: 0.044, OR-3.26, CI-1.1-10.8) and marital conflicts (P: 0.003, OR-3.2, CI-1.4-6.9) by the end of second trimester; history of miscarriages (P: 0.012, OR-2.58, CI-1.2-5.4) by the end of third trimester; lower SES (P: 0.001, OR-3.48, CI-1.64-7.37), unsatisfied living conditions (P: 0.004, OR-2.9, CI-1.4-6.04), alcohol use in husband (P: 0.049, OR-2.01, CI-1.1-4.11), history of depressive episodes (P: 0.049, OR-2.09, CI-1.1-4.46), history of high-risk pregnancy (P: 0.008, OR-2.7, CI-1.29-5.64), history of miscarriages (P: 0.049, OR-2.04, CI-1.1-4.2), stressful events in the postpartum period (P: 0.043, OR-2.58, CI-1.01-6.59), IUD (P: 0.002), preterm birth (P: 0.001), congenital malformations (P: 0.001), dissatisfaction with the sex of the child (P: 0.005, OR-3.75, CI-1.42-9.91), poor family support (P: 0.001), and low birth weight (P: 0.001, OR-16.78, CI-6.32-44.53) in the postpartum period. These analyses are purely exploratory. Conclusions: PND is highly prevalent from the early antenatal period onwards; this warrants periodic assessment of depression among high-risk mothers, using a validated tool, for early diagnosis and management.

Currently, the risks posed by bacteria are becoming increasingly important. It now appears that the cell wall of Anammox image bacteria is very different from what has been generally considered for many years. Not every textbook contains the peptidoglycan on the cell wall of Anammox image bacteria - the sugar-protein chain that strengthens the cells of most bacteria. Most researchers in this Anammox image bacteria diseased identification wanted to find out what gave the Anammox image cell its stability. It used powerful cryo-electron microscopes to examine the bacterial cell wall and find the exact structure of the peptidoglycan. A new algorithm is proposed to discover that Anammox image bacteria contain peptidoglycan, which completes a theory in microbiology. The identification of different diseases is listed, and the proposed model compares the exact results while comparing the parameters like accuracy, precision, recall, and F1-Score. RESUMO

Chorioangiomas are the most common non-trophoblastic benign vascular tumor of the placenta, highly associated with perinatal death rate. Herewith, we are reporting the prenatal diagnosis, management and postnatal outcome of a fetus referred at 33 weeks gestation with massive cardiomegaly secondary to placenta chorioangioma.

Introduction: Proteinuria is recognized as an independent risk factor for cardiovascular disease in kidney transplant recipients, but previous studies have not considered the impact of changes in urine protein over time. Research Question and Design: We used time-dependent, multivariable Cox proportional hazards models in this observational cohort study of adult kidney transplant recipients to evaluate whether proteinuria measured by dipstick on random spot urine samples starting from 1-month post-transplant was associated with the risk of major adverse cardiac events and graft loss. Results: A total of 144 major adverse cardiac events, defined as acute myocardial infarction, cerebrovascular accident, revascularization, or all-cause mortality, were observed in 1106 patients over 5728.7 person-years. Any level of proteinuria greater or equal to trace resulted in a two-fold increase in the risk of major adverse cardiac events (hazard ratio 2.00 [95% confidence interval 1.41, 2.84]). This relationship was not found to be dose-dependent (hazard ratios of 2.98, 1.76, 1.63, and 1.54 for trace, 1+, 2+, and 3+ urine protein, respectively). There was an increased risk of graft failure with greater urine protein concentration (hazard ratios 2.22, 2.85, 6.41, and 19.71 for trace, 1+, 2+, and 3+, respectively). Conclusion: Urine protein is associated with major adverse cardiac events and graft loss in kidney transplant recipients. The role of interventions to reduce proteinuria on decreasing the risk of adverse cardiovascular and graft outcomes in kidney transplant recipients requires further study.

This report describes two cases of tachycardia-induced cardiomyopathy secondary to incessant ectopic atrial tachycardia (EAT) in an infant presenting with severe left ventricular dysfunction and hemodynamic instability. The two cases were managed differently. The first required mechanical ventilation and was resistant to conventional antiarrhythmic drugs. After the initiation of enteral ivabradine (0.15mg/kg) the heart rate slowed with significant improvement in hemodynamics, peripheral perfusion and sinus rhythm was restored after 12 hours. Ivabradine was continued and the patient was discharged home after 10 days of hospitalization. The second case was managed by early initiation of ivabradine and resulted in restoration of sinus rhythm within 4 hours, thus avoiding trials of conventional anti-arrhythmic drugs with unstable hemodynamic profile. The infant was discharged after 5 days of hospitalization on ivabradine..

Although classically considered a cornerstone of inpatient care, rounding at patients' bedsides is increasingly being replaced by rounding in workrooms. Workroom rounds may provide a sense of efficiency and comfort, however bedside rounds have multiple benefits for patients, trainees, and staff physicians. Alongside its benefits, there are human and institutional challenges when incorporating bedside rounding. This article aims to draw on our own experience of implementing bedside rounding at Kingston Health Sciences Centre, to guide staff physicians and institutions on how to implement bedside rounding effectively while overcoming its challenges. The following seven tips provide a framework to avoid pitfalls when implementing bedside team rounding on inpatient services.

First described in 1965 by Johnson and Wilcox, partial absence of the tricuspid valve (TV) associated with a ventricular septal defect (VSD) is a very rare malformation (Ann Surg. 1966;164:334-336). Heart failure secondary to severe TV regurgitation results in utero fetal compromise (J Am Coll Cardiol. 1991;17:167). We present here a fetus with partial absence of the tricuspid valve with perimembranous VSD.

The development of membrane-based technologies for the treatment of wastewater streams and resources containing heavy metal ions is in high demand. Among various technologies, nanofiltration (NF) membranes are attractive choices, and the continuous development of novel materials to improve the state-of-the-art NF membranes is highly desired. Here, we report on the synthesis of poly(homopiperazine-amide) thin-film composite (HTFC)-NF membranes, using homopiperazine (HP) as a monomer. The surface charge, hydrophilicity, morphology, cross-linking density, water permeation, solute rejection, and antifouling properties of the fabricated NF membranes were evaluated. The fabricated HTFC NF membranes demonstrated water permeability of 7.0 ± 0.3 L/(m2 h bar) and rejected Na2SO4, MgSO4, and NaCl with rejection values of 97.0 ± 0.6, 97.4 ± 0.5, and 23.3 ± 0.6%, respectively. The membranes exhibit high rejection values of 98.1 ± 0.3 and 96.3 ± 0.4% for Pb2+ and Cd2+ ions, respectively. The fouling experiment with humic acid followed by cross-flow washing of the membranes indicates that a flux recovery ratio (FRR) of 96.9 ± 0.4% can be obtained.

Aortic atresia is uncommonly associated with atrioventricular and ventriculoarterial discordance. Presence of severe regurgitation of Ebstein malformation of the tricuspid valve in this subset results in reduced aortic blood flow in-utero. We report here a term neonate with this anomaly detected antenatally.

BACKGROUND: Insight is awareness of one's symptoms, illness, and need for treatment. In bipolar disorder, insight is better in bipolar depression and nonpsychotic depression when compared with mania and psychotic depression. Insight impacts on medication adherence. In our study, we measured and compared with various clinical parameters of insight. The aim of this study is to assess various dimensions of insight recovery prospectively in bipolar affective disorder patients with treatment and drug compliance. MATERIALS AND METHODS: Patient's insight was assessed using Mood Disorder Insight Scale (MDIS) at baseline, 1st, 3rd, and 6th months. Their insight was then compared with various sociodemographic profiles and correlated with number of mood episodes, family history of mental illness, and 6-month MDIS scores. RESULTS: Depression patients scored better in insight components (P = 0.001). The good compliance group attributed their symptoms to their illness than the poor compliance group (P = 0.013). The MDIS scores were gradually improving from baseline to 6 months (P ≤ 0.001). There was no relationship between insight and the number of episodes (P = 0.788). CONCLUSION: Depressive episode patients had better insight during the baseline, which improved during 6 months follow-up compared with manic patients. Among various components of insight, insight on the attribution of symptoms was a predictor of good compliance. Progression of insight was steady and proportionate to the duration of treatment in depressive episode patients.

OBJECTIVE: To analyze the sequelae of Neer type 2 distal end clavicle fractures treated with superior anterior locking plate. METHODS: From June 2012 to February 2015 a study was conducted with 14 male patients; 12 with unilateral and two with bilateral distal clavicle fractures treated with superior anterior plate. They were evaluated at intervals, with mean follow up of 16 months (14-18 months). All patients were evaluated clinically by both the Oxford Shoulder Score and the QuickDASH score. RESULTS: Union was seen in all fractures within 7-9 weeks (mean time: 8.2 weeks). All patients had good shoulder range-of-motion. The average Oxford Shoulder Score and QuickDASH score were 45.6 and 7.1, respectively. All patients returned to work within 3-4 months of the postoperative period. CONCLUSION: Displaced distal clavicle fractures treated with superior anterior plates accomplished superlative results in terms of bony union, with rarely any complications.

BACKGROUND: Femoral shaft fractures are an incapacitating pediatric injury accounting for 1.6% of all pediatric bony injuries. Management of these fractures is largely directed by age, fracture pattern, associated injuries, built of the child and socioeconomic status of the family. We retrospectively evaluated the use of elastic stable intramedullary nail (ESIN) in surgical management of femoral shaft fractures in children and its complications. METHODS: Fifty two children were treated with titanium elastic nails (TEN) from June 2009 to June 2014 at our institution. At the end of the study there were 48 children. Fractures were classified according to Winquest and Hansen's as Grade I (n=32), Grade II (n=10), Grade III (n=6) and compound fractures by Gustilo and Anderson's classification, Grade I (n=5), Grade II (n=3). There were 36 mid-shaft fractures, 7 proximal third shaft fractures, 5 distal third shaft fractures. The final results were clinically evaluated by using Flynn's criteria and radiologically by Anthony et al's criteria. RESULTS: The mean duration of follow-up was 20 months (range 12 - 40 months). All fractures healed radiologically with grade III callus formation at 9 - 12 weeks (mean 9.7 weeks). The results were analyzed using Flynn's criteria and were excellent in 40 children (83%) and satisfactory in 8 children (17%). The soft tissue discomfort near the knee produced by nail ends was the most common problem in our study (25%). Other complications include limb shortening (n=5), Varus malunion (n=4), Nail protruding site infection (n=4) and nail migration (n=2). There was no delayed union, non-union or refractures. CONCLUSION: TEN is minimally invasive, safe, relatively easy to use and an effective treatment for fracture shaft of femur in properly selected children.Level of evidence: III.

OBJECTIVE: Analyze the outcomes of proximal femoral locking compression plate (PF-LCP) in these complex fractures. METHODS: This study retrospectively analyzed 21 proximal femoral fractures treated with PF-LCP from June 2013 to February 2015. There were 15 females (71%) and six males (29%) with an average age of 61.4 years (range: 34-80 years). The peritrochanteric fractures constituted by intertrochanteric and subtrochanteric fractures were classified by the Boyd and Griffin classification and Seinshemier's classification, respectively. Among them, 16 cases (76%) were of intertrochanteric and five cases (24%) were of subtrochanteric fracture pattern. The functional outcome was assessed by Harris Hip Score and the Parker Palmer mobility score one year post-surgery. RESULTS: Among 21 patients, 19 patients obtained fracture union without further intervention; two patients required additional bone grafting. No cases of the hip screw cutting the femoral head were noted. There was no post-operative mortality in this study. The average Harris Hip Score was 84.5 (range: 83-94). The assessment by Parker and Palmar mobility score was 7.5 (range: 4-9). CONCLUSION: The PF-LCP is a good, stable alternative in the treatment of peritrochanteric femoral fractures. It provides good-to-excellent bone healing with reduced complications.


OBJETIVO: Analisar os resultados da placa de compressão bloqueada do fêmur proximal (PF-LCP) nestas fraturas complexas. MÉTODOS: Este estudo retrospectivamente analisou 21 fraturas proximais do fêmur tratadas com PF-LCP entre junho de 2013 e fevereiro de 2015. Foram incluídas 15 mulheres (71%) e seis homens (29%) com idade média de 61,4 anos (intervalo: 34 a 80 anos). As fraturas peritrocantéricas constituídas por fraturas intertrocantéricas e subtrocantéricas foram classificadas pela classificação de Boyd e Griffin e pela classificação de Seinshemier, respectivamente. Entre elas, 16 casos (76%) foram classificadas como padrão intertrocantérico e cinco casos (24%) como padrão subtrocantérico. O resultado funcional foi avaliado pelo escore de quadril de Harris e pelo escore de mobilidade de Parker Palmer um ano após a cirurgia. RESULTADOS: Dentre os 21 pacientes, 19 obtiveram união de fratura sem intervenção adicional e dois pacientes necessitaram enxerto ósseo adicional. Nenhum caso de corte da cabeça femoral pelo parafuso do quadril foi observado. Não houve mortalidade pós-operatória neste estudo. A média do escore de quadril de Harris foi de 84,5 (intervalo: 83 a 94). A média do escore de mobilidade de Parker Palmer foi de 7,5 (intervalo: 4 a 9). CONCLUSÃO: A PF-LCP é uma alternativa adequada e estável no tratamento de fraturas femorais peritrocantéricas, propiciando uma osteossíntese classificada como boa ou excelente, com poucas complicações.

OBJECTIVE: The aim of the study is to evaluate the clinical and functional outcome in treatment of infected tibial nonunion by the Ilizarov method. METHODS: The authors retrospectively analyzed 42 patients with infected tibial nonunion with bone loss; shortening and deformity treated at this institution during the period of February 2012 to April 2015 were included in the study. The results were evaluated according to Association for the Study and Application of the Methods of Ilizarov (ASAMI) criteria. Pin tract infections were assessed by Moore and Dahl score. RESULTS: Bone results were excellent in 60% of patients (n = 25), good in 15% (n = 6), fair in 25% (n = 11), and poor in none. The functional results were excellent in 55% of patients, good in 30%, fair in 5%, and poor in 10%. The most common complication found in this study was pin tract infection. CONCLUSION: Ilizarov ring fixator still remains an excellent treatment modality for tibial nonunion, as it addresses the problems associated with the condition, although the procedure is cumbersome.


OBJETIVO: O objetivo do estudo é avaliar o desfecho clínico e funcional no tratamento da pseudartrose infectada da tíbia pelo método de Ilizarov. MÉTODOS: Os autores analisaram retrospectivamente 42 pacientes com pseudartrose infectada da tíbia com perda óssea. Os casos de encurtamento e deformidade tratados nesta instituição durante o período de fevereiro de 2012 a abril de 2015 foram incluídos no estudo. Os resultados foram avaliados de acordo com os critérios da Associação para o Estudo e Aplicação dos Métodos de Ilizarov (Association for the Study and Application of the Methods of Ilizarov [ASAMI]). As infecções no trajeto dos fios e pinos foram avaliadas pela classificação de Moore e Dahl. RESULTADOS: Os resultados ósseos foram excelentes em 60% dos pacientes (n = 25), bons em 15% (n = 6) e moderados em 25% (n = 11); nenhum paciente apresentou resultados ruins. Os resultados funcionais foram excelentes em 55% dos pacientes, bons em 30%, razoáveis em 5% e ruins em 10%. A complicação mais comum encontrada neste estudo foi infecção no trajeto dos fios e pinos. CONCLUSÃO: Embora seja um procedimento complicado, a fixação de Ilizarov continua a ser uma modalidade de tratamento excelente para pseudartrose da tíbia, uma vez que aborda os problemas associados à condição.

ABSTRACT OBJECTIVE: The aim of the study is to evaluate the clinical and functional outcome in treatment of infected tibial nonunion by the Ilizarov method. METHODS: The authors retrospectively analyzed 42 patients with infected tibial nonunion with bone loss; shortening and deformity treated at this institution during the period of February 2012 to April 2015 were included in the study. The results were evaluated according to Association for the Study and Application of the Methods of Ilizarov (ASAMI) criteria. Pin tract infections were assessed by Moore and Dahl score. RESULTS: Bone results were excellent in 60% of patients (n = 25), good in 15% (n = 6), fair in 25% (n = 11), and poor in none. The functional results were excellent in 55% of patients, good in 30%, fair in 5%, and poor in 10%. The most common complication found in this study was pin tract infection. CONCLUSION: Ilizarov ring fixator still remains an excellent treatment modality for tibial nonunion, as it addresses the problems associated with the condition, although the procedure is cumbersome.


RESUMO OBJETIVO: Avaliar o desfecho clínico e funcional no tratamento da pseudartrose infectada da tíbia pelo método de Ilizarov. MÉTODOS: Os autores analisaram retrospectivamente 42 pacientes com pseudartrose infectada da tíbia com perda óssea. Os casos de encurtamento e deformidade tratados nesta instituição de fevereiro de 2012 a abril de 2015 foram incluídos no estudo. Os resultados foram avaliados de acordo com os critérios da Associação para o Estudo e Aplicação dos Métodos de Ilizarov (Association for the Study and Application of the Methods of Ilizarov [ASAMI]). As infecções no trajeto dos fios e pinos foram avaliadas pela classificação de Moore e Dahl. RESULTADOS: Os resultados ósseos foram excelentes em 60% dos pacientes (n = 25), bons em 15% (n = 6) e moderados em 25% (n = 11); nenhum paciente apresentou resultados ruins. Os resultados funcionais foram excelentes em 55% dos pacientes, bons em 30%, razoáveis em 5% e ruins em 10%. A complicação mais comum encontrada neste estudo foi infecção no trajeto dos fios e pinos. ABSTRACT OBJECTIVE: Analyze the outcomes of proximal femoral locking compression plate (PF-LCP) in these complex fractures. METHODS: This study retrospectively analyzed 21 proximal femoral fractures treated with PF-LCP from June 2013 to February 2015. There were 15 females (71%) and six males (29%) with an average age of 61.4 years (range: 34-80 years). The peritrochanteric fractures constituted by intertrochanteric and subtrochanteric fractures were classified by the Boyd and Griffin classification and Seinshemier's classification, respectively. Among them, 16 cases (76%) were of intertrochanteric and five cases (24%) were of subtrochanteric fracture pattern. The functional outcome was assessed by Harris Hip Score and the Parker Palmer mobility score one year post-surgery. RESULTS: Among 21 patients, 19 patients obtained fracture union without further intervention; two patients required additional bone grafting. No cases of the hip screw cutting the femoral head were noted. There was no post-operative mortality in this study. The average Harris Hip Score was 84.5 (range: 83-94). The assessment by Parker and Palmar mobility score was 7.5 (range: 4-9). CONCLUSION: The PF-LCP is a good, stable alternative in the treatment of peritrochanteric femoral fractures. It provides good-to-excellent bone healing with reduced complications.


RESUMO OBJETIVO: Analisar os resultados da placa de compressão bloqueada do fêmur proximal (PF-LCP) nessas fraturas complexas. MÉTODOS: Este estudo retrospectivamente analisou 21 fraturas proximais do fêmur tratadas com PF-LCP entre junho de 2013 e fevereiro de 2015. Foram incluídas 15 mulheres (71%) e seis homens (29%) com média de 61,4 anos (34 a 80). As fraturas peritrocantéricas constituídas por fraturas intertrocantéricas e subtrocantéricas foram classificadas pela classificação de Boyd e Griffin e pela classificação de Seinshemier, respectivamente. Entre elas, 16 casos (76%) foram classificados como padrão intertrocantérico e cinco (24%) como padrão subtrocantérico. O resultado funcional foi avaliado pelo escore de quadril de Harris e pelo escore de mobilidade de Parker Palmer um ano após a cirurgia. RESULTADOS: Dentre os 21 pacientes, 19 obtiveram união de fratura sem intervenção adicional e dois necessitaram de enxerto ósseo adicional. Nenhum caso de corte da cabeça femoral pelo parafuso do quadril foi observado. Não houve mortalidade pós-operatória neste estudo. A média do escore de quadril de Harris foi de 84,5 (83 a 94). A média do escore de mobilidade de Parker Palmer foi de 7,5 (4 a 9). CONCLUSÃO: A PF-LCP é uma opção adequada e estável no tratamento de fraturas femorais peritrocantéricas, propicia uma osteossíntese classificada como boa ou excelente, com poucas complicações.

BACKGROUND: Quality of life has become an important aspect in the measurement of the health of an individual as the population ages. Rhythm-centred music making (RMM) has been shown to improve physical, psychological and social health. The purpose of this study was to explore the effects of RMM on quality of life, depressive mood, sleep quality and social isolation in the elderly. METHODS: A randomised controlled trial with cross over was conducted. 54 participants were recruited with 27 participants in each arm. In phase 1, group A underwent the intervention with group B as the control. In phase 2, group B underwent the intervention with group A as the control. The intervention involved 10 weekly RMM sessions. Patient related outcome data which included European Quality of Life-5 Dimensions (EQ5D), Geriatric Depression Scale (GDS), Pittsburg Sleep Quality Index (PSQI) and Lubben Social Network Scale (LSNS) scores were collected before the intervention, at 11th and at the 22nd week. RESULTS: A total of 31 participants were analyzed at the end of the study. The mean age was 74.65 ± 6.40 years. In analysing the change in patient related outcome variables as a continuous measure, participation in RMM resulted in a non-significant reduction in EQ5D by 0.004 (95% CI: -0.097,0.105), GDS score by 0.479 (95% CI:-0.329,1.287), PSQI score by 0.929 (95% CI:- 0.523,2.381) and an improvement in LSNS by 1.125 (95% CI:-2.381,0.523). In binary analysis, participation in RMM resulted in a 37% (OR = 1.370, 95% CI: 0.355,5.290), 55.3% (OR = 1.553, 95% CI: 0.438,5.501), 124.1% (OR = 2.241, 95% CI = 0.677,7.419) and 14.5% (OR = 1.145, 95% CI = 0.331,3.963) non-significant increase in odds of improvement in EQ5D, GDS, PSQI and LSNS scores respectively. CONCLUSION: Participation in RMM did not show any statistically significant difference in the quality of life of the participants. It is however, an interesting alternative tool to use in the field of integrative medicine. Moving forward, a larger study could be performed to investigate the effects of RMM on the elderly with an inclusion of a qualitative component to evaluate effects of RMM that were not captured by quantitative indicators. TRIAL REGISTRATION: This trial was retrospectively registered. This trial was registered in the Australian New Zealand Clinical Trials Registry under trial number ACTRN12616001281482 on 12 September 2016.

RESUMO

We investigated a multifacility outbreak of acute hepatitis B virus infection involving 21 residents across 10 assisted living facilities in Texas during the period January 2008 through July 2010. Epidemiologic and laboratory data suggested that these infections belonged to a single outbreak. The only common exposure was receipt of assisted monitoring of blood glucose from the same home health care agency. Improved infection control oversight and training of assisted living facility and home health care agency personnel providing assisted monitoring of blood glucose is needed.
